In 4821 medical students who participated in the Examination Cremesp between 2005 and 2011, 2,250 were not approved, a failure rate equal to 46.7%. That is, among all the students who were about to enter the market - and that took the tests - nearly half were considered unprepared for the profession, according to minimum criteria established by the Regional Council of Medicine of São Paulo.

The Examination Cremesp, optional up to 2011, evaluates the performance of graduates of medical stringers of São Paulo. Under the supervision of the Cremesp at all stages, the test is applied by the Carlos Chagas Foundation, an institution with extensive experience in procurement.

The Cremesp established as the cutoff for approving the settlement of 60% of the questions. In all years, the trials were composed of 120 questions divided by nine basic areas of medical knowledge. By this criterion, were approved students who scored at least 72 questions.

The examination of 2011, the latest series of seven consecutive years, was held last October. Of the 418 students who participated, 191 of them - or 46.0% of the total - less than 72 questions answered correctly. It means, as determined by Cremesp who are not prepared for the profession.

In four of the seven years of tests, more than half the students failed. In 2008, only 39% of 730 participating students agreed minimum of 72 questions. In 2009 and 2007 the percentage was 44% approved. The best results, with 62% and 68% approval was obtained in 2005 and 2006, years in which the examination was still in the experimental stage. Over the past five years, the average approval was always less than 60%. This trend is consistently regarded as unsatisfactory by Cremesp and worrying.

The outcome of the Cremesp shows, so persistent, serious deficiencies in medical education in the State of Sao Paulo. It is important to consider that the situation could be worse. Due to the optional test, theoretically better prepared students may demonstrate greater interest in participating in the evaluation.

The refusal of the students can be deduced that the notes would be more worrying if there was greater participation

Because it is voluntary, and the consequent irregularity in the participation of schools, the distribution of participants was not homogenous among the courses. The small number of students from various schools does not allow to evaluate the performance of individual institutions. It is therefore possible to establish a performance ranking of schools. The refusal of the students can be deduced that the notes would be more worrying if there was greater participation. Students and institutions that do not feel prepared, they tend not to expose themselves before exams are not mandatory. One goal of the examination mandatory from 2012 is just down the distortion and allow a more comprehensive and thorough medical education in Sao Paulo.

The Examination Cremesp now mandatory, settles a historic position of the Council of St. Paul who always supported and participated in several experiments evaluating the courses and their graduates. In the 1990s, for example, headed the project Cinaem - National Interagency Commission for the Evaluation of Medical Education. More recently, it has supported the measures of the MEC to reduce the number of vacancies, on the poor results of medical schools in the National Student Performance, Enade. The Cremesp advocates the establishment of a National Qualification Examination, which depends on approval of a law by Congress.
