The Hospital Miguel Couto in Rio de Janeiro, a reference in vascular surgery, is implementing new protocols of care for patients with diabetic gangrene. From now on, the hospital hopes to multidisciplinary care to these patients to perform the operation. The unit receives on average per day, three patients with necrosis due to clogging of the arteries caused by diabetes.

The new protocols were developed by the Center for Hospital Quality and Access (NAQH), set in action by the SOS Emergency Unit of the Ministry of Health, which is run in partnership with states and municipalities to qualify the management and care in major emergencies that meet the SUS.

The protocols will be applied from the host of the patient in an emergency. These patients typically stay seven to ten days hospitalized for treatment with antibiotics and specific examinations. Only then can undergo bypass surgery, to try to save part of the limb is not affected by gangrene, and amputation of the necrotic area.

According to the director of the Hospital Miguel Couto, Luiz Alexandre Essinger, despite the expansion of actions and tests offered in the primary care network for the diagnosis and treatment of diabetes, many people are unaware of having the disease and seek medical care only to realize necrosis (spoilage) of the fabric, usually of the toes when the amputation as is inevitable.

Equipment will be purchased with funds from the SOS Emergency, as two new x-ray machines - C-arm - help in vascular surgery as the patient can be operated. Also plotted a profile of the users, showing where they see if they had previously diagnosed diabetes and who did not seek treatment earlier. The report containing this information will assist the development of new strategies in the core network to provide early treatment to those frames.

There is also the possibility of including these patients in the program of Home Care (AD), reducing the average length of postoperative hospital stay, now about 15 days a month. Thus, the dressing changes, the fundamental healing period, shall be done at home by the team and family AD qualified. Currently, Miguel Couto has the Home Care Program for the Elderly (PADI), the Municipal Health Department, serving in the house about 40 vascular patients a total of 150 users. With the Best Home program, the action of the Ministry of Health also provides house calls, the number of people with vascular problems can be amplified.

The hospital is strengthening the vascular surgery outpatient clinic for follow-up of people who have suffered amputation and require review and constant care to prevent the occurrence of new necrosis.

SOS Emergency Health Network integrates the whole time, and besides Miguel Couto, was deployed in the State Hospital Albert Schweitzer, both in Rio de Janeiro. In all, at this stage, we selected 11 hospitals, all references are regional, have emergency room and perform a large number of hospitalizations and outpatient visits. The goal is that by 2014 the SOS Emergency reach the top 40 Brazilian emergency rooms in 26 states and the Federal District.
